Client Onboarding Assessment

Welcome to Project Got Your Back Navigator Program.

 

Purpose-Audience

The purpose of the Project Got Your Back (PGYB) Navigator Program is to provide a Veteran Navigator to a Transitioning Military Veteran client to direct and assist the veteran with need resources to help guide client through their transition into civilian life.

This series will help to transition military members navigate their opportunities for medical, education, and career decisions, as well as develop the person as a whole to be equipped for civilian life. PGYB is aware that many military transition programs do not provide adequate resources and structure needed to transition successfully, and we built this program to serve those who have served us.

At this phase beginning the Navigator Program we ask our Veteran Clients to take two separate assessments designed to help the Veteran assess areas of their transition which need he most and immediate attention.

The Results of these assessments are used in three ways.

  • First we help our Veteran Clients become aware of their immediate needs.
  • We capture the data in the program to “blindly measure” the improvements of all the veterans in the PGYB Navigator program. This data is critical for PGYB to obtain funding in order to provide this service to Veterans at no cost. The assessment scores are not displayed to PGYB staff in a manner they can be tied to a specific Veteran’s name.
  • Finally, we ask our Veteran Clients to share the results of these assessments with their Navigator.  This allows the Navigator to most efficiently and effectively deliver service to our clients.
  • SHARING OF THE ASSESSMENT RESULTS IS DONE COMPLETELY AT THE DISCRETION OF THE VETERAN CLIENT.
  • Please email your assessment results to your Navigator.

The Expected Investment:

The relationship  with your PGYB Navigator is designed to last 12 months.  We are aware that in most cases relationships will continue past the formal program term as well as  some may be completed sooner if the specific needs of the Veteran Client are met.

The Navigator and Client should interact as often as necessary in order to complete a successful Veteran transition, securing all of the required services and resources needed by the Veteran.  Just as your time in the service there are plenty of tasks which are your responsibility. If referrals to services are made, it is expected that you will do your part and connect with these services as directed.

 

The Outcomes:

 

  • The transitioning military member will have formed a valuable relationship.
  • The transitioning military member will  be directed by someone who has traveled a similar path.
  • The transitioning military member will gain more clarity about their benefits.
  • The transitioning military member will be directed to available resources to meet their needs.
  • The transitioning military member will gain more confidence in discussing his or her experience/career with others.
  • The transitioning military member will have closed transition resource and service gaps.
  • The transitioning military member will have gained additional introductions to build their network and gain a new community.
