In addition to the PRISM Networking platform, the PGYB Navigator also utilizes a community resource referral platform provided by Unite Us. The benefits and objectives of this platform are quite important to our organization, our clients, and the service organizations who we refer our clients to for valuable services.
By using Unite Us we begin to close the gaps which have existed in the past where someone being referred to a support agency were not promptly given needed services. The UniteUs platform is a case management system which allows the participating agencies to document referrals for service and provide accountability that the services were performed. Not only do we improve service referral outcomes, but another important benefit of the platform is that all the service partners in the community network can capture the “referral” data and measure the effectiveness of the entire partner network. This data drives organizational improvement and will play an important roll going forward for organizations such as PGYB to document our impact and to continue to raise money to fund our program.
As the Unite Us platform is more broadly adopted by more service partners in Minnesota more of the service organizations we make referrals to will come on line. It is important to track the “out of network” referrals in Unite Us so we can use that data to encourage organizations who are not participating in Unite Us to come on board.
Following your successful completion of the Navigator certification course a PGYB staff member will create a log in for you to use the Unite Us platform.
